All New and Returning Student-Athletes:
Another season of Garrett College athletics is just around the corner, and we’re excited to have you as part of Laker Nation!
Before you may particpate in any athletic activity, including use of the athletic facilities for GC Athletics, you must have the following on file in the Athletic Training Office:
1) PHYSICAL PACKET
Must be dated after July 1, 2026 and the GC Athletics Physical Form must be completed.
No other physcial form will be accepted.

2) INSURANCE
All student athletes are required to have Primary Insurance. You must provide proff of insurance cards and information. Failure to do so will result in No Athletic Participation.
A copy of the front and back of you insurance card is required.

Students may also be eligible for state subsidized or state funded insurance in the state of residency.

You may choose to bring with you at the start of the semester to turn in. (No Athletic Activity will be allowed until all forms and proof of insurance are on file in the Athletic Training Department.)
All sports are encouraged to send ahead of time to ensure a prompt start of the season.
Fall Sports - All paperwork must be submitted by August 10, 2026 to ensure prompt participation in the pre-season.
3) PRE-PARTICIPATION FORMS
Before you can participate in athletics, you must complete the online Athletics Pre-Participation Packet in the My.GarrettCollege student portal. This packet is required each academic year.
Before you begin, please gather the following information:
- Emergency contact information
- Insurance card or policy information
- Physician or medical clinic info (if you have one)
To complete the packet:
- Log in to my.garrettcollege.edu using your Garrett College username and password.
- Select Student.
- Select Online Forms.
- Scroll down to the Athletics Online Forms section.
- Select Athletics Pre-Participation Packet.
Please submit the packet as soon as possible. If you have trouble accessing the form, contact our office for assistance.

Visiting Teams:
The Athletic Training room, #966, is located inside the weight room, #965, across the hall from the gymnasium in the CARC. The Athletic Training room will open approximately 1 hour prior to game time, and will stay open 30 minutes after the conclusion of the sporting event.
The following will be available to all teams at each of our home sporting events:
- Licensed Athletic Trainer
- Pre - game taping (please provide supplies)
- Ice injury/post-game
- Emergency equipment including AED
